Our Bristol Emergency Department
As a busy inner-city ED, based in the largest teaching hospital in the South West, our Bristol ED cares for over over 60,000 patients, 24 hrs a day, 365 days a year. Within the department there are over 40 doctors, 100 nurses and around 40 support staff.
Our Bristol ED recently won BMJ and HSJ awards as well as being recognised in two categories at the RCEM awards. With a focus on innovation and wellbeing, the ‘Shine’ ED Patient Safety Checklist, widely used across the NHS, was also developed by our staff right here in Bristol.
Our Weston Emergency Department
Our Weston Emergency Department is based in the beautiful coastal town of Weston-super-Mare and cares for approximately 55,000 patients per annum. Needless to say it can be a very busy but rewarding department to work in! You will be joining an experienced team delivering a full range of minor injury and urgent care services from 8am until 10pm. We are also able to offer additional financial incentives and training opportunities (including 80:20 specialist interest post) on top of our standard relocation package for doctors looking to join our Weston division.

Same Day Emergency Care (SDEC)
The UHBW Same Day Emergency Care (SDEC) department offer a patient pathway that increases efficiency, reduce unnecessary admissions, reduce overcrowding in the ED and improve the patient experience.
SDEC is a transformational change in care that supports the provision of same day care for emergency patients who would otherwise be admitted to hospital. Under this model, patients presenting at hospital with relevant conditions can be rapidly assessed, diagnosed and treated without being admitted to a ward; and if clinically safe to do so will go home the same day their care is provided. A fast-paced and innovative place to work which will give clinical staff variety and opportunities to develop.

ED Nursing Fellowships (Coming soon…)
UHBW have agreed to support a pilot project, offering backfill for a pre-approved period of leave, to allow nursing fellows to deploy to Nanyuki alongside medical fellows.
What is involved?
- 10 week deployment to Nanyuki in Kenya
- Support educational projects (eg developing teaching systems/programs)
- Support quality improvement within the ED (eg care pathways, ECG use)
- Support broader health system development
- Flexible working pattern but generally working weekdays while in Nanyuki with leave opportunities

SOPHIE’S STORY
Clinical Fellow in Emergency Medicine
Meet Dr Sophie Brattan SHO – 12 months at BRI as Clinical Fellow in Emergency Medicine and Expedition Medicine.
Where have you been?
My time was split between working in the BRI Emergency medicine team and Nepal as an expedition medic was one of the most exciting and educational years I have had in my short career in Medicine. In Nepal I was based out of Kathmandu volunteering for a UK charity ‘Raleigh International’ providing medical care and advice to the staff and youth who volunteer with the charity.
As a department, the BRI ED team became one big family to me, especially throughout our difficult winter months. The senior clinical team are very enthusiastic with teaching. We received dedicated time to attend teaching, undertook SIM days, had teaching on the shop floor, and also had opportunities for audits/posters which have all strengthened my skills and my future applications for specialty ED training. Staff wellbeing is highly recognised and we even went surfing as a staff body in the Spring months!
Lieutenant Colonel Harvey Pynn has many contacts and was supportive in helping us secure some experience in expedition medicine and ensure we had appropriate indemnity.
What were the challenges
and memorable moments?
In regards to my time in Nepal, I had many memories of being the field medic accompanying 20 young people (International and Nepali) on a 20 day trek through the Himalayan foothills!
Challenges included, leech bites, diarrhoea outbreaks amongst the group, and a few emergencies.
A particular case involved a non-diabetic experiencing hypoglycaemia and me treating her in a valley basin where we had little phone signal. As the only medical professional on the trek, I was multitasking treating her, directing colleagues to find signal to communicate the situation with our medical team back in the office but also to make judgement call to evacuate her.
What did you learn?
The case above highlights how your clinical skills were challenged but also your logistical skills, leading a team of non-medical professionals and working in environments with little resources. Furthermore, when working with new cultures, I had the challenge of treating patients who don’t immediately trust you as a western doctor and compliance was a common issue.
Would you recommend the post?
I would highly recommend taking this post as you step away at the end of the year with a broader range of skills than I could have anticipated, lifelong friends and a lot of stories.

Global Health
At the core of the Global Health Fellowship is the opportunity to spend 2 months alongside staff in Nanyuki ED, developing clinical practice and governance. The project builds on previous work by ‘Future Health Africa’ in the region and looks to develop Emergency Medicine as a speciality in the region.
Activities will involve development projects and co-ordinating a formal training course provided by BRI ED staff. These posts fit recognised frameworks for Global Health posts through ‘Future Health Africa’ and RCEM.
Including comprehensive personal, academic and clinical development through structured local training, funded postgraduate accreditation in global health, and overseas deployment.
You will receive 24-hour support and ‘top cover’ UK supervision, and support with travel and accommodation logistics.
Expedition Medicine
Expedition medicine will test you and push you to your limits, professionally, physically and emotionally but our doctors say it’s worth it. Working as a team is key to success in this exciting discipline. Expedition opportunities at UHBW include:

- Support you to complete RCEM Core USS competencies
- Support cost of undertaking PGCert Gen
- Med USS (or PGD Specific Modules if already trained to PGC level)
- Offer additional USS experience in main radiology service (1day per week)
- Provide exposure/training in enhanced USS modalities if/when
- Core completed. If/when core competencies achieved to contribute to USS training in dept and on National course faculty.

Search and Rescue
This 12 month post offers the opportunity to train and subsequently deploy, on a voluntary off rota basis, with the Severn Area Rescue Association (SARA). Activities include land based search and rescue, cliff rescue, swift water rescue, cave rescue and flood rescue.
Your own transport would be essential with activities taking place out of various regional bases including Beachley in Monmouthshire.

Frailty
The following could be offered individually

- 20% Frailty – working alongside the new acute frailty service assessing frail older people in ED and AMU
- 20% Orthogeriatrics – working alongside the team to assess elderly trauma patients and contribute to their ongoing management (silver trauma)
- 20% Stroke – working on the acute stroke unit and being part of the team performing assessments for thrombolysis / thrombectomy.

- Work with South Western Ambulance Service NHS Foundation Trust Learning and Development Team. Contribute to the Trust training needs analysis and creation of guidance and clinical guidelines
- Deliver training to SWASFT learning and development officers
- Observe shifts with SWASFT clinical units such as double crewed ambulances, rapid response vehicles and the hazardous area response team.

Community Emergency Medicine Service
Our Bristol Community Emergency Medicine is looking to recruit Emergency Medicine ST4 equivalent (or higher) with a specific interest in community care and pathway development. 20% of their time would be dedicated to Community Emergency Medicine; working with the Specialist Paramedics in Urgent and Emergency Care, responding to the whole spectrum of prehospital cases and assisting crews in assessment and treatment on scene.
This is not a critical care service but one that looks to support best care to patients in the most appropriate place and pathway.
The role involves both clinical delivery, education, collaborative working with SWAST, governance and quality improvement.

Remuneration
The time pursuing specialist interest is paid
at the same rate as clinical work. Travel costs will need to be covered by the individual. You will also receive NHS Benefits and full holiday entitlement.
Entry requirements
The posts have the option to be 100% ED or split 80/20% and are open as Out Of Programme Training / Experience for EM Higher Specialty Trainees and as stand-alone posts for non-trainees. We are seeking highly motivated applicants for Clinical Fellow posts (which can be OOPT/E or stand-alone posts) at ST1-3 and ST4+ level in Emergency Medicine. Our ST1-3 posts are not exclusive to emergency medicine trainees, we will accept applications with all levels of experience and interest.

If you are currently in training. You will need to request permission from your current deanery, to join us. This is a usually a straightforward process, if completed in early.
We advise you speak to your Training Programme Director at your earliest convenience to discuss our OOPT/E opportunity.
