NHS@Home is an innovative new virtual ward service, providing care for people who are acutely unwell in their own homes across Bristol, North Somerset and South Gloucestershire. It offers an alternative to hospital and helps people return home from hospital more quickly, too.
The service uses a mixture of face-to-face visits, phone and video calls, and remote monitoring through digital technology, to help people recover safely in the comfort of their own home.
NHS@Home has six pathways, covering a wide range of conditions including frailty, respiratory, heart failure, and general surgical recovery. Our team provide a very wide range of acute healthcare treatments within the home, in addition to monitoring health conditions and vital signs for deterioration.
The service is run in partnership between Sirona care & health, North Bristol Trust, University Hospitals Bristol and Weston NHS Foundation Trust and BrisDoc.

Services like NHS@Home are at the forefront of the NHS. They combine the very latest innovation and technology with holistic, person-centered – and face to face – community care. Working in our team you will learn a very wide acute care skills in a community setting. You’ll also contribute to a multi-disciplinary approach, undertaking board rounds and working alongside consultants, advanced care practitioners, nurses and therapists to devise effective treatment plans that make a real difference to the lives of local people.

You may also want to volunteer, perhaps as a driver, to drop off medications, or being a ‘digital friend’ to give people a call or check kit and equipment. The NHS in partnership with the Royal Voluntary Service are looking for an ‘army’ of volunteers who can support the people in England who have long term health conditions (and/or are at most risk from covid) to stay well.
